What Skilled Nursing Visits Can Include

Services depend on the individual’s current orders, clinical needs, payer requirements, and NNHC’s ability to safely provide the requested intervention.

Clinical Assessment

Focused nursing assessment, monitoring, and documentation related to the ordered plan of care.

Medication Support

Medication administration, teaching, or other nursing interventions when ordered and within scope of practice.

Wound Care

Ordered wound assessment, dressing changes, treatment, and monitoring when appropriate to the referral.

Caregiver Teaching

Instruction and reinforcement for family caregivers on ordered home-care routines when included in the plan.

Change-of-Condition Reporting

Recognition and communication of clinically significant changes to the appropriate provider or care team.

Other Ordered Nursing Needs

Additional skilled services may be considered when medically appropriate, ordered, and within NNHC’s current capabilities.

Intermittent Skilled Nursing vs. Private Duty Nursing

These services are not interchangeable.

Skilled Nursing Visits

Typically involve shorter, intermittent nursing visits for a specific ordered clinical need, assessment, treatment, teaching, or monitoring.

Private Duty Nursing

Provides extended one-on-one RN or LPN coverage for individuals with ongoing complex skilled needs requiring more continuous nursing support.

Common Referral Situations

Families and referral sources often contact NNHC when a person needs skilled follow-up at home after a change in condition or treatment plan.

After Hospital or Facility Care

Ordered nursing support after discharge when the person still needs skilled monitoring or treatment at home.

Wound & Skin Concerns

Ordered wound-care services when ongoing treatment, documentation, and clinical monitoring are needed.

Medication or Clinical Changes

Nursing support when a new or changed treatment plan requires licensed assessment, teaching, or administration.

Coverage & Authorization

Skilled nursing coverage depends on medical necessity, payer rules, current orders, and authorization requirements.

NC Medicaid Home Health includes medically necessary skilled nursing services. South Carolina Medicaid also maintains home-health nursing benefits under its home-health program. Other payers, including VA-authorized care and private-pay arrangements, may have different documentation and authorization requirements. NNHC reviews the payer source and referral before confirming service.

The specific skilled service being requested, frequency, treatment instructions, and relevant clinical information.

Payer Information

Authorization, referral source, plan or program information, and any service-specific payer requirements.

Location & Scheduling

Where care is needed, requested frequency, preferred schedule, and any time-sensitive start-of-care needs.
