Personal Care Services at Home

Respectful, hands-on support with everyday activities so individuals can remain safer, more comfortable, and more independent at home.

What Personal Care Can Include

The exact tasks depend on the person’s assessed needs, service plan, payer authorization, and the scope of the assigned caregiver.

Bathing & Grooming

Hands-on assistance with bathing, oral care, grooming, dressing, and other personal-hygiene routines.

Toileting & Incontinence Care

Support with toileting routines, hygiene, and continence-related care within the service plan.

Mobility & Transfers

Assistance with walking, repositioning, and transfers when included in the plan and within caregiver competency.

Eating & Routine Meal Support

Help with eating and routine meal-related assistance when allowed by the service plan and any swallowing precautions.

Cueing & Supervision

Reminders, redirection, and safety oversight for individuals who benefit from structured support.

Routine Household Support

Light, care-related household tasks that directly support the client’s health, safety, and daily routine when included in the plan.

Who May Benefit

Personal care can support adults with disabilities, older adults, people living with cognitive impairment, and others who need hands-on help with activities of daily living.

In North Carolina, Medicaid Personal Care Services are tied to an independent assessment and individualized service plan. In South Carolina, personal care is also used within Medicaid home- and community-based service pathways. NNHC does not determine Medicaid eligibility; we review referrals and authorizations after the appropriate payer or program determines coverage.

Personal Care Is Not Skilled Nursing

Clear service boundaries help families choose the right level of care.

Personal Care

Focuses on activities of daily living, routine assistance, cueing, supervision, mobility support, and other non-skilled tasks included in the care plan.

Skilled Nursing

Includes licensed nursing assessment and interventions such as certain medication administration, wound care, airway management, enteral care, and other clinical tasks ordered within a nursing plan of care.

Payment & Authorization

NNHC works with multiple payer types, but coverage rules are not the same for every client.

NC Medicaid

PCS eligibility and authorized services are determined through NC Medicaid and its assessment/authorization process.

SC Medicaid

Personal care may be available through applicable South Carolina Medicaid and HCBS programs when the individual meets program requirements.

VA & Private Pay

Some clients receive VA-authorized home-care services or arrange qualifying services privately, subject to NNHC acceptance and staffing.

What Happens After You Contact Us

We do not promise staffing before we understand the case.

1. Initial Conversation

We learn the general location, schedule, payer, and type of help being requested.

2. Referral Review

We review the authorization or service documents and determine whether NNHC can safely and realistically support the case.

3. Start-of-Care Planning

If accepted, we confirm the service plan, caregiver needs, schedule, and next steps before services begin.

Service notice: Personal care availability, covered tasks, authorized hours, eligibility, and start dates vary by payer, program, location, client needs, and staffing. NNHC confirms service only after referral review and acceptance.
