Wound Care Must Follow Current Orders
Home wound care is a skilled nursing service when the treatment requires licensed assessment or intervention.
NNHC reviews the wound-care order, diagnosis, treatment instructions, dressing products, frequency, documentation requirements, payer authorization, and whether appropriately trained nurses are available before accepting the referral.
Examples of Wound & Clinical Support
The exact service is based on the individual plan of care and the nurse’s authorized scope.
Wound Assessment
Observation and documentation of wound characteristics, surrounding skin, drainage, pain, and clinically significant changes.
Dressing Changes
Ordered dressing changes and topical treatments using the products and schedule specified by the treating provider.
Clinical Monitoring
Monitoring for changes that may require provider notification or reassessment of the treatment plan.
Caregiver Teaching
Instruction and reinforcement on ordered wound-care or related routines when appropriate to the plan.
Coordination With Providers
Communication of relevant findings to the ordering provider, wound clinic, or other authorized members of the care team.
Related Skilled Needs
Other clinical interventions may be reviewed when they are ordered, medically necessary, and within NNHC’s service capabilities.
When a Wound Needs More Than Routine Home Care
Some wounds require specialty evaluation, urgent assessment, or a higher level of care.
Increasing redness, swelling, odor, drainage, pain, fever, tissue changes, uncontrolled bleeding, exposed structures, rapidly worsening skin breakdown, or other concerning changes should be handled according to the person’s treatment and escalation plan. Emergency symptoms require emergency evaluation.
Coverage & Medical Necessity
Skilled nursing coverage generally depends on medically necessary services, current orders, and payer requirements.
NC Medicaid Home Health includes medically necessary skilled nursing, and its current clinical policy specifically addresses skilled nursing and complex wound-care needs. South Carolina Medicaid also covers skilled nursing through its home-health program. NNHC verifies the applicable payer requirements and authorization before confirming visits.
