Free In-Home Assessment

An in-home assessment is scheduled with our healthcare professional (such as a nurse, social worker, or case manager) or senior agency representative. This meeting takes place at your home to determine a person’s requirements in order to create a personalized care plan, assess the individual’s physical, emotional, social, and environmental needs.

Preliminary Conversation:

  • One of our team members will interview the patient and/or their family or caregiver about their medical history, daily routine, and concerns.
  • During this visit we will collect your Primary Care Physcian’s information as they are part of your care.

Physical Assessment:

  • After our discussion we will collect vitals, and ask you to demostrate a few movements or tasks.
  • This gives us an idea of youe health, mobility, and physical limitations.
  • We will also review of contemporary medical treatments, prescription medicines, and chronic ailments.

Daily Living Activities:

  • Assessing the person’s ability to do ADLs, such as:
    • Showering, dressing,
    • Meal Prep, eating,
    • Toileting, incontinence
    • Mobility, Ambulation (like getting out of bed)
    • Medication Reminder and adminstration
    • Light Housekeeping, shopping

Cognitive and Emotional Health:

  • We will assess for anxiety, depression, memory issues, and cognitive impairments.\

Home Environment:

  • We will assess the home for safety hazards such loose carpeting
  • Inadequate lighting
  • Grab bars and house safety and accessibility improvements.

Social and Emotional Needs:

  • Recognizing the person’s friends, family, and local resources. Assessing their mental health and loneliness.

Caregiver Needs:

  • A caregiver’s stress, capacity to care for others, and need for support or rest are also assessed.

Recommendations and Planning:

  • Based on the results, the care provider creates a personalized care plan.
  • This includes organizing certified in-home caregivers.
  • Advising on medical or house improvements.
